When should I freeze eggs with endometriosis?

The short answer

Consider egg freezing earlier if endometriosis symptoms are progressing, ovarian reserve is reduced, or ovarian surgery is planned. Timing also depends on age and pregnancy plans; adenomyosis alone typically doesn’t reduce egg supply.

With endometriosis, consider egg freezing earlier if symptoms are progressing, ovarian reserve testing suggests time sensitivity, or surgery for an endometrioma is planned, since ovarian surgery can reduce egg supply. There is no universal starting age, including at 25: your age, fertility goals, and individual disease trajectory matter, with greater time sensitivity in your mid-30s or older or when surgery, recovery, treatment, or life circumstances will delay pregnancy.

Adenomyosis itself typically doesn’t reduce egg supply, although age and coexisting endometriosis can affect fertility, and freezing eggs does not treat adenomyosis or its effects on the uterus. Hormonal stimulation for egg freezing may temporarily worsen endometriosis symptoms, but evidence about lasting disease progression is limited.

Our comprehensive, whole-body evaluation includes prior imaging, operative reports, fertility-related labs, and ultrasound findings to assess ovarian reserve and the potential effects of cysts, inflammation, or previous procedures; we also consider PCOS, thyroid dysfunction, tubal issues, male-factor infertility, and uterine conditions because these can change the fertility strategy. Insurance coverage, including Medicare, depends on your country, plan, and eligibility, and an endometriosis diagnosis alone does not guarantee coverage.

We can help weigh egg or embryo freezing against surgery timing and coordinate assisted reproduction, including IVF, when appropriate. If you’d like individualized planning for fertility preservation and symptom control, you’re welcome to contact our team for a consultation.

Questions people ask next

All fertility questions
  • Can I freeze eggs if I have an ovarian endometrioma?

Yes, many people can still pursue egg freezing with an ovarian endometrioma, but the plan should be individualized. The cyst can make monitoring and retrieval more challenging and may affect follicle count, so cyst size, symptoms and ovarian reserve help guide the plan.

  • Will ovarian cystectomy improve fertility?

Ovarian cystectomy can improve fertility in some situations, especially when a cyst disrupts ovulation, distorts pelvic anatomy, or drives inflammation. However, ovarian surgery can also reduce ovarian reserve, depending on how much healthy ovarian tissue is affected.

  • Egg freezing vs embryo freezing with endometriosis: which is better?

With endometriosis, the better choice usually depends on your sperm source and future plans. Embryo freezing often gives the clearest picture of what you’ve preserved, while egg freezing preserves flexibility if your plans, relationship status, or sperm choice could change.

  • Will egg retrieval worsen endometriosis?

For many patients, egg retrieval and IVF stimulation do not appear to dramatically accelerate endometriosis in the short term. Temporary pain flares are common, and limited data suggest stimulation may contribute to progression in some cases of deep infiltrating endometriosis.
