Can endometriosis cause IBS-like symptoms without bowel lesions?

The short answer

Yes, endometriosis can cause IBS-like symptoms even without visible bowel lesions on imaging or at surgery. Pelvic inflammation, heightened nerve sensitivity, hormone-driven changes, and pelvic floor muscle overactivity can all contribute to bowel symptoms.

Yes. Many patients with endometriosis experience IBS-like symptoms even when no bowel lesions are visible on imaging or at surgery. Inflammation in the pelvis, heightened nerve sensitivity, and hormone-driven changes can all affect how the gut functions and feels, leading to bloating, constipation, diarrhea, cramping, and pain.

Pelvic floor muscle overactivity can also mimic or amplify bowel symptoms by making it harder to empty comfortably and increasing pressure and pain with digestion or bowel movements. In our practice, we look at the whole pattern (cycle timing, pelvic pain triggers, bowel symptoms, and exam findings) to identify what’s driving your symptoms and which treatment approach is most likely to help. If these symptoms sound familiar, you can reach out to schedule a consultation with our team to discuss a focused evaluation and personalized plan.

Questions people ask next

All symptoms questions
  • Bowel symptoms that flare with periods: IBS or bowel endometriosis?

Bowel symptoms that flare with periods can suggest bowel endometriosis, but IBS can also worsen during menstruation. Ovulation-related flares, worsening period pain, or pain with sex add clues; IBS and endometriosis can coexist, so timing alone cannot distinguish them.

  • Why do some GI doctors say endometriosis can't cause bowel symptoms?

Some GI doctors say this because their training focuses on problems inside the digestive tract. Endometriosis can affect the bowel’s outer surface, deeper wall, and nearby nerves, causing bowel symptoms even when colonoscopy results look normal.

  • IBS vs IBD vs endometriosis: how can I tell the difference?

IBS often causes fluctuating bloating and bowel changes, while IBD is more likely with blood in the stool, fevers or weight loss. Endometriosis can mimic both, with clues like painful bowel movements, deep pelvic pain and bowel changes that worsen before or during your period.
