Can adenomyosis affect fertility or pregnancy?

The short answer

Yes, adenomyosis can affect fertility and pregnancy, although not everyone will struggle to conceive. It may interfere with implantation and is associated with higher risks of miscarriage and preterm birth, especially with more diffuse disease.

Yes, adenomyosis can affect both fertility and pregnancy, although not everyone with it will struggle to conceive. Endometrial-like tissue within the uterine muscle can alter uterine contractions, increase inflammation, and reduce the uterine lining’s receptivity, contributing to infertility and recurrent implantation failure. Adenomyosis is also associated with higher risks of miscarriage and preterm birth.

Fertility impact depends on disease location, pattern, and extent, with diffuse adenomyosis often having a greater impact than focal disease, while commonly coexisting endometriosis can compound fertility challenges. There is no single pregnancy rate that applies to everyone with adenomyosis.

We start with a careful symptom history and high-quality imaging, often ultrasound and/or MRI, to clarify the disease pattern and tailor a plan to your pregnancy goals. Depending on your situation, options may include medical pretreatment, adjustments to an IVF approach, or uterus-sparing treatment, including targeted removal of a focal, accessible adenomyoma (adenomyomectomy) and other conservative approaches in selected cases.

If you’re trying to conceive or planning pregnancy, you’re welcome to schedule a consultation with our team to understand what your imaging may mean and coordinate next steps that prioritize fertility and pregnancy safety.

Questions people ask next

All fertility questions
  • Can focal adenomyosis affect fertility or miscarriage risk?

Yes, focal adenomyosis can affect fertility and miscarriage risk, especially when a lesion distorts the uterine cavity or interferes with implantation. Not all focal adenomyosis behaves the same: smaller nodules farther from the cavity may have less impact.

  • Can adenomyosis cause miscarriage?

Adenomyosis may contribute to miscarriage for some patients, but many people with adenomyosis have healthy pregnancies. Changes in the uterine muscle and environment may affect implantation and early placental development, and risk varies from person to person.

  • How does adenomyosis affect an IVF plan?

Adenomyosis can make implantation less predictable and increase the risk of certain pregnancy complications. IVF planning may include medical suppression before transfer and a frozen embryo transfer strategy, tailored to your symptoms, imaging findings, and prior IVF history.

  • When is adenomyomectomy recommended, and how does it affect pregnancy?

Adenomyomectomy is typically considered when focal adenomyosis causes significant pain or heavy bleeding despite medical treatment, grows, or may affect fertility. Some patients are advised to allow healing before pregnancy and may be counseled toward cesarean delivery because of uterine repair.
