Diagnosis
Can imaging confirm endometriosis?
The short answer
Imaging can be very helpful, but it usually can’t confirm endometriosis on its own. Ultrasound or MRI may strongly suggest disease, but a normal scan doesn’t rule it out because many small, superficial or microscopic lesions won’t show up on scans.
Imaging can be very helpful, but it usually can’t confirm endometriosis on its own. Ultrasound or MRI may strongly suggest disease when they show findings such as endometriomas (ovarian cysts related to endometriosis) or deep infiltrating lesions.
However, many endometriosis lesions are superficial, small, or microscopic and won’t show up on scans even when symptoms are significant. A normal ultrasound or MRI doesn’t rule out endometriosis, and an abnormal scan is best interpreted alongside your symptoms and exam. Questions people ask next
All diagnosis questionsHow is endometriosis diagnosed, and do I need surgery?
Endometriosis diagnosis starts with symptoms, a pelvic exam, and expert imaging; surgery is not automatically required to begin care. Laparoscopy with biopsy remains the gold standard for definitive diagnosis and can allow treatment at the same time.
Can endometriosis be present with normal ultrasound and MRI?
Yes, it’s very possible to have endometriosis even when ultrasound and MRI are normal. Scans can miss superficial implants, small lesions, or disease in less-visible areas, so normal imaging should not automatically rule it out when symptoms persist.
Will endometriosis show up on ultrasound?
Sometimes, but a normal ultrasound does not rule out endometriosis. Ultrasound is very good at finding ovarian endometriosis cysts and, with specialist imaging expertise, can detect many cases of deep disease; superficial lesions are rarely visible.
Will endometriosis show up on MRI?
Sometimes, but a normal MRI does not rule out endometriosis. MRI is best at picking up endometriosis-related ovarian cysts and deep disease, but superficial endometriosis and small implants often don’t show up, even when symptoms are significant.
