Can adenomyosis cause constipation?

The short answer

Yes, adenomyosis can contribute to constipation even without bowel endometriosis. An enlarged, tender uterus, period-related inflammation, and pelvic floor muscle guarding may make bowel movements harder, but other causes can overlap.

Yes, adenomyosis can contribute to constipation even when endometriosis isn’t growing on or in the bowel. An enlarged, tender uterus and period-related inflammation and cramping can create pelvic pressure that makes bowel movements harder, especially before or during your period.

Pain and pressure can also trigger pelvic floor guarding, where muscles tighten subconsciously to protect against pain, making it harder to relax and empty fully and leading to straining or incomplete evacuation. Constipation and straining can contribute to hemorrhoids or worsen them, rather than adenomyosis directly causing hemorrhoids.

Constipation isn’t specific to adenomyosis: bowel endometriosis, IBS-like bowel sensitivity, and pelvic floor dysfunction can overlap, and many people have adenomyosis and endometriosis together. Tracking constipation around bleeding, along with bloating, rectal pressure, painful bowel movements, cyclical pelvic pain, heavy bleeding, deep aching, or pain with sex, helps clarify what needs evaluation.

Treatment depends on the contributing causes and may include managing adenomyosis, measures to ease constipation, and pelvic floor physical therapy when appropriate. If you’d like help understanding the connection, contact our team to schedule a consultation so we can evaluate overlapping causes and discuss a treatment plan that fits your goals.

Questions people ask next

All deep disease questions
  • Can constipation be a sign of bowel endometriosis?

Yes, constipation can be a sign of bowel endometriosis, especially when it predictably flares before or during your period. It can result from effects on the bowel, adhesions or pelvic floor muscle spasm, but symptoms don’t reliably show how deep endometriosis is.

  • Can adenomyosis cause pain with bowel movements?

Yes, adenomyosis can cause pain with bowel movements. Because the rectum sits directly behind the uterus, uterine cramping and pressure can be felt as rectal pain even when the bowel itself is not affected. Painful bowel movements can also have overlapping causes.

  • Can endometriosis cause IBS-like symptoms without bowel lesions?

Yes, endometriosis can cause IBS-like symptoms even without visible bowel lesions on imaging or at surgery. Pelvic inflammation, heightened nerve sensitivity, hormone-driven changes, and pelvic floor muscle overactivity can all contribute to bowel symptoms.

  • Why does adenomyosis cause bloating?

Adenomyosis can cause bloating because cyclical swelling and inflammation can make the uterus enlarged and tender, even without endometriosis. This can create abdominal fullness or distension, especially before and during your period.
